====== Fitness ====== This consent form is only part of the process of informed consent. It should give you the basic idea of what physiological testing and training entails and what your participation will involve. Please take the time to read this carefully and to understand any accompanying information. If you would like more detail about something mentioned here, or information not included here, please ask. The following physiological tests may be conducted to generate and track your physiological profile overtime: Resting Blood Pressure and Heart Rate Your resting blood pressure will be measured using a sphygmomanometer and a stethoscope and your resting heart rate will be measured for 15 seconds using a stethoscope or radial palpation. These measures are used to determine your readiness to participate in activity. Height and Weight Height (Standard and/or Stretch) and Weight Shuttle Run You will run back and forth on a 20-m course to a set protocol (e.g. AIS, Leger, etc.). The stage speeds will increase in a linear fashion. The test is terminated when you fail to reach the end line two consecutive times. The test is maximal in nature and performance. Heart rate may be monitored by a telemetric device or taken manually by radial pulse or stethoscope. Core Stability Plank Test You will hold a plank position for a maximum of 3 minutes. During that time you will be asked to keep a straight line from your ears to your toes. At different time intervals you will be asked to lift one or more limbs off of the ground. Total Time will be recorded. Push Ups The body is positioned such that the back is straight, and the feet are dorsiflexed (or on the knees ) the hands are palm down and in a sport specific width apart . The movement must be smooth with time evenly spent in the upward and downward phases. The push-ups may be performed to a rate set by a metronome. Pull Ups Using a specified grip (underhand, reverse or neutral) you will grasp the pull-up bar with hands approximately shoulder width apart. You will then pull yourself up until your chin is just above the level of the pull-up bar and then descend to full arm extension. You will complete as many repetitions as possible while maintaining good form. Vertical Jump (Power) You will perform vertical, squat and/or countermovement jumps up to five times each with 5-15sec rest. Flight time, peak force, maximal vertical height, L/R asymmetries will be recorded by a jump measurement system (E.g. Opto-jump, Force Plate System, Vertec etc.). Jumps may be performed bilaterally and/or unilaterally. If training at CSI Alberta, you will be under the supervision of a qualified professional. They will provide guidance and program training on elements of strength and conditioning including warm up, musculoskeletal range of motion and loading patterns, cardiovascular work, cool down and recovery modalities as appropriate. Data may be collected during your training sessions to track progress and inform future program design. There have been few, if any, complications for those participating in the above stated physiological testing and training. If you are not tolerating the stress, the activity will be stopped immediately. Mild light-headedness and muscle soreness may occur but should disappear quickly. Risk of injury is possible in all physical activity but is minimal. Any injuries or feeling of discomfort should be reported to the qualified professional immediately. If you suffer an injury as a result of participating in the above stated physiological testing or training, no compensation will be provided to you by the Canadian Sport Institute Alberta. The qualified professionals are trained in emergency procedures and have the appropriate qualifications and certification to conduct the above stated physiological testing and training. You still have all your legal rights. Nothing said here about treatment or compensation in any way alters your right to recover damages. Your physiological and training data will be compiled and stored in a centralized database and will be used to generate and track your physiological profile over time. Access to the database is secure, firewalled protected, confidential and strictly controlled under the direction of the Canadian Sport Institute Alberta Sport Science Committee. Your results may be released to your sport organization, coach or designated IST or parent/guardian (if under the age of majority). The CSI Alberta will do its best to make sure that your private information is kept confidential. Information about you will be handled as confidentially as possible, but there is always the potential for an unintended breach of privacy. Your signature on this form indicates that you have understood to your satisfaction the information regarding your participation in physiological testing and training and acknowledge that this informed consent form is valid for a maximum of 12 months from the date it is completed. In no way does this waive your legal rights nor release the Canadian Sport Institute Alberta from their legal and professional responsibilities. You are free to withdraw from the physiological testing and training at any time without jeopardizing your health care.
